Does anyone have intake sheets they use in their Parenting groups? We utilize intake sheets and give to each mom to fill out prior to seeing Pedi MD, questions normally the MA would ask in traditional care are on this intake sheet? We are wanting to revise ours and looking for ideas from others sites that may use something similar.
I have attached a copy of our 2 month intake sheet.
