Here is a chart review tool our providers use before groups. I agree with what was already said- it is much smoother if you can take the time to review charts before group. Using this sheet, we can see how many people will need labs, etc. so no one sneaks out without getting what they need. I am also attaching a vitals sheet that the patient fills out at the beginning of group before they see the provider. Most of our providers then take notes on this sheet, keep it, and chart in the EMR later. It makes the belly checks much quicker, and you don't have to fuss if the internet stops working in the group room, as sometimes happens to us! patient vitals assesment sheet for group 1....docx (18.18 KB) Prenatal chart review template.docx (30.09 KB)

I really like the one the patient fills out. On mine I added a "to do this visit" line to be sure everything is completed before the patient goes home.

This is the "shadow chart" we developed at Grady. We had a EMR but it was not in the Centering Space. This sheet allowed us to review the EMR and prep for group anywhere we had a computer. This sheet holds protected information so must be locked and stored in a secure place.

Centering Pregnancy Visit Log VITAL SIGN W....xlsx (13.85 KB) Aloha, Friends! Here is our "visit log" for charting VS and for providers to write notes to themselves. I "think" patients feel pretty good about having free access to their "record" and being able to see their results and the plan of care. During 1:1 time, I am using the computer and inputting the VS in the EMR (we have two systems), unless I need to facilitate some kind of ice-breaking activity. Mahalo! P.S. The providers often get the charts from me to review prior to the session, unless they keep their own notes somewhere. They do their charting in the 30-60 minutes following the session as I do the physical cleanup/tear down.

I have a prep landing worksheet that I can send to you. Simply, there needs to be a pre planning session set aside to review the charts with your co-facilitator. It can not be done during group time . I will send you my template. We started using this worksheet in January and our providers really like it. We put all prenatal labs done before first centering group then continue to add to the sheet. The patients leave the sheet with us each time . It has saved a lot of work and time. Also we know right away what labs need to be done. We do write ins for any extra labs that have been done at the top of the sheet on the right hand column. This is the kind of sharing that we had hoped for when we started CenteringConnects. Thank you all for sharing your work with the rest of us! I am sure many sites will benefit from this. And perhaps you all have seen some new ideas from the others here that you could incorporate in your own forms and documents.

I am interested in Centering because: I want to make a difference in someones life for the better and help educate them about the better way of healthcare. I am passionate about what I do with our Centering Program at Kaiser Permanente. I love the lifelong friendships that I build while going thru the journey of pregnancy for all of my patients. I love the personal care that patients receive and I love to keep our groups fun and current. Coming up on being accredited for a year in Jan 2018 and we have grown so much. Already starting 2 groups a month starting Dec 2017, beat our goal to start 2 by a month. #MicDrop

If anyone is on Epic , I would like to share that i have asked my other Co-Facilitators to use the Pre-Visit worksheet, it has any Health Maintenance alerts, it has a spot for the patient to put their Height Weight and Blood Pressure and is also has the last 3 Blood Pressures and Weights of the patient, it has their last PHQ9 since we are doing them so often now during pregnancy, this part is very helpful, the patient doesn't know what that is nor do they ask, I highlight BP and Weight for the pt and I review the patients charts before Centering and discuss with the midwife, any open labs, any labs that are due for the patient or if they need their US ordered ect. I feel the more work we as co-facilitators take off of the midwives the more the midwives can strictly focus on the patient and running a smooth and successful group! It eases stress on all areas, and the patients don't fall thru any holes or have to come back to the clinic a million times because of our fault. It makes it a better experience for all and I have asked my co facilitators to follow my flow so it is the same throughout our clinic. It is working so well!!!! Love my Centering Team !!!!!

I am interested in Centering because: As a military spouse myself, I can see the HUGE benefit Centering has to our expectant moms who are often many miles from home with no friends or family nearby! I feel like the Centering model is a perfect fit for our environment!

Hello All! Here is my Centering Group Worksheet. You can save it and make adjustments as you need or want after you save it to your system. I have found that basically, Sessions 1-4 are the prenatal visits that have the most needs i.e. labs, ultrasounds, individual special health needs which I put in under "other needs" One important point is that is document falls under HIPAA and will need to be protected. I was making just a generic "table" with a column for the patient's name and their EGA and then a larger one for any notes the provider may want to make during their one-on-one time (ie. needs labs, US, meds, etc,) but I like your much better! You can make any file a PDF by opening it and using the "print as" function - choose Adobe PDF. I hope that helps you save these great resources!

Not if you don't have access to MS Office applications. However, there are a few other solutions 1. If you have access to Google Docs, you can upload the .DOC file and convert to a Google Doc file during upload. You can then select the file in Google Docs and download the file as a PDF. 2. You can also upload the .DOC file to Google Docs without converting it. Then double click on the file in Google Docs to open the preview, select the printer icon to open the print dialog, select PDF as your destination and follow the prompts to save. This also works for .PPT files. 3. There are online web applications that can help you with the file conversion. Simply search "convert .doc to .pdf".
