I am interested in Centering because: I am the Centering Pregnancy and Parenting Manager and Midwifery Program Manager. I am also the Centering Community Coordinator for Cuyahoga County.

As I am cleaning out my desk, I realize I have piles of signed confidentiality and photo release forms from past groups. What does everyone do with these? How long should they be kept? I don't want to scan them into our EMR unless it is necessary. Any information is helpful.

Hi Alison, CHI doesn't offer any guidance around this because it really falls under your organization's legal policies. I would suggest discussing your options with your legal department. They will need to know how and why the confidentiality and photo release forms are used and also if you are using photos of your patients in social media, web site, or other promotional uses.
