we have been doing Centering group intakes for 2 months now. We run 1 per week and have a maximum of 10 women enrolled. We developed a questionnaire that the patient completes at the UPT visit and we have time to set up a dummy chart for patients. At group intake they get 1 on 1 time with the provider who would be their centering provider to review those papers and discuss private concerns, trying to keep that to 3 minutes per person. During that time the co facilitator will do one of the activities to keep people engaged. After all the one on one visits are done the provider joins for a more discussion and activities prior to having pt. schedule NMV (with that provider) and 1st or 2nd trimester screening if applicable. We close the deal so to speak at the NMV. We also have snacks during this time. We have noticed that our groups have been increasing in size so it might be working!! Too soon to tell, more to come. We started doing Centering Intake Groups at our site this summer. We are doing 1-2/month now. We schedule 6 patients for each group. 3 patients arrive 1 hour before the Intake Group and rotate through 3 stations: RN (for vitals, pregnancy test (if needed), history, blood draw); registered dietitian; and social worker. Then they attend the Intake Group and leave after. The other 3 patients come 15 minutes before the Intake Group for vitals and a pregnancy test (if none previously documented). They go to the Intake Group and then after they rotate through the 3 stations.
The Intake Group itself is scheduled for 45 minutes and done by myself (a CNM). We have the patient sign a confidentiality agreement. We have snacks and water.We review our patient education folder and discuss topics (using a facilitative approach) such as: Nausea/vomiting relief measures When/how to call for pregnancy issues
At the end we introduce Centering and discuss how it works, answer questions, give out Centering ink pens.

this sounds very interesting! We have not thought of doing 50% upfront then stagger the arrival time and the other 50% at end to do the parts that need more 1 on 1 attention! Great thinking, I am going to present this to my group!!

Our facility is looking to try out the group intake idea. We are a growing facility and would like to know what people have done to get started. Our biggest concern is how we will manage to get the health and history from the patients. Is there a set of forms patients are doing that is making it work? We get the whole intake group process and goal, but we are worried about the flagging of higher risk patients if we do an all opt in at first.
Any suggestions or ideas will be great!

I am passionate about what I do with our Centering Program at Kaiser Permanente. I love the lifelong friendships that I build while going thru the journey of pregnancy for all of my patients. I love the personal care that patients receive and I love to keep our groups fun and current. Coming up on being accredited for a year in Jan 2018 and we have grown so much. Already starting 2 groups a month starting Dec 2017, beat our goal to start 2 by a month. #MicDrop

Is there any forms people have available to share in regards to obtaining that personal patient information? How do the flow of the groups work?
