Here is a look into the proposal scoring process:
Motivation
- The Readiness Assessment (RA) is an auto-calculation and the same as for any site that submits an RA, whether they are applying for the implementation grant or not.
- Other areas that will be auto-scored are if the site is in a target state (OH, KY, IN, MI, IL, WI, MN, MS, LA, AR, TX, OK, NM), if it is urban / suburban / rural, and practice setting.
- The subjective areas from the open-ended questions fall into three categories: motivation, population and commitment. Full points will be awarded for the following scenarios:
Motivation
- Specific process or outcomes goals align with the listed goals of the initiative, clear motivation for CenteringParenting and other identified models
- The scenario is specific and realistic from the site's starting point, the case for the change desired is well-articulated
Population
- Baseline/regional data provided to give picture of health quality in the community and articulate need
- Baseline/regional data provided for multiple factors influencing the social determinants of health, need articulated
- Needs and health impact assessments completed recently and results described, already engaged with community partners
- Sufficient familiarity with model and patient culture to understand and articulate why group care would be successful
- There is a vision beyond CenteringPregnancy & CenteringParenting, beginning of a plan described
Commitment
- Effective Steering Committee already in place (current practice) and involved in submission of proposal, composition described and representative (includes CHC board member and patient rep); for new sites committee composition is described and is representative, involvement in proposal submission described
- Leaders and their roles are identified and well-described, and are active on the committee.
