I am interested in Centering because: I am the new CenteringPregnancy Nurse Lead for a large OB/midwife office in Southwest Michigan.

Hello, Our providers have begun a discussion on whether additional weekly visits after 36 weeks, as in the traditional model of care, are necessary within Centering for low risk women. We are curious if other Centering groups around the country are foregoing these extra visits, or still incorporating them with Centering. I have been doing some research on the necessity of them outside of Centering and have so far found that there is usually a higher risk of intervention with a greater number of prenatal visits. I would love some feedback on what other groups are doing at this time. Thanks!

Our patients do come in for individual appointments the weeks between centering groups once they are around 36-37 weeks pregnant. I also think it would be interested to hear what others are doing.

Our moms come in for individual visits on the off weeks from Centering after 36 weeks too.

Our patients come in for individual clinic visits in between their Centering visits toward the end!

We don't have any routine extra visits at the end of pregnancy -- but neither do we see low-risk women in individual care more than every two weeks until 40 weeks. We would start seeing them for individual care at 41 weeks. I think you are right that the evidence doesn't support more frequent visits.

I have seen many sites that do Individual visits on the in between weeks for everyone; sites that only see moms every two weeks and I have also seen sites add group visits to make them weekly after 36 weeks. Seems it has a great deal to do with peoples comfort level as their is no absolute rule. When I was doing Centering - back in the day - I found that it was much easier for me to see my Centering moms every 2 weeks in group and skip the in between visit as I knew them so much better then I knew moms in individual care (for the most part) Mind you, there were often people whose late pregnancy symptoms made me bring them in for additional appointments, but this can happen at anytime, not just late pregnancy.
