"Community Health Workers and Clinical Integration" Slides

Hi All:  Here are the slides presented today on the Circle Up Webinar presented by Jaye Clement from Henry Ford Health System in Detroit on their WIN program integrating Community Health Workers into their enhanced CenteringPregnancy groups.  The webinar was recorded.  It takes about a week to transpose the recording onto our website through the Portal in the Pre-recorded webinars, so look for it toward the end of next week.  It was really a super presentation.
