Hello,
Has anyone created a digital format of the Centering Pregnancy/Parenting patient evaluation that they are willing to share? How are you collecting the evaluations?
I was thinking of created a google doc survey like the ones passed out in our webinars but wanted to see what others have tried.

I created a google doc - copying the questions on the form in to Google. I added some questions about doing groups Virtually. I wanted to see what patients feedback on Zoom session was. You can see mine here: forms.gle/yfgxCk1wQgsDFQnh8 |

Hello,
I'm having trouble collecting evaluations from our Virtual CenteringPregnancy patients. I send out the evaluations via email to the patients and ask them to return it either by email or give it to the Midwife or nurse at her next appointment. I've also had their Centering nurse ask for the evaluations to be turned in. Unfortunately, patients aren't returning them. Is anyone else experiencing the same issue? If not, what are some strategies you've used to get patients to return the evaluations? Thanks! Helen

Hi everyone! My practice has been mailing out the surveys with a pre posted and addressed envelope. I have had a better turn around time when giving incentives; such as $10 amazon gift card once its received. I have gotten the quickest and largest turn around with incentives.
