We have quite a few factories in our area. I am finding that a certain number of patients are having difficulty getting to office visits in general or getting the time off to come to Centering. The factories seem to be very strict about time off and giving "points" if someone needs an appointment. I have a few questions that I would appreciate feedback on. 1. Has anyone hosted an evening Centering group? 2. If so, did you find there was a high no show rate or did the patients attend regularly? 3. Was there a certain time that seemed to work better? 4. Also, has anyone addressed Centering with employers in their community? 5. If so, what was the outcome?

Hi Patti,
There are many, many practices that have successful evening groups. Most started them because enrollment or attendance was low for daytime groups due to work and childcare. I hope some fellow practices chime in soon and if I come across some good examples I will post them here!

We had very good attendance and usually always had a partner there3. Was there a certain time that seemed to work better? We met 6 to 8 pm. We had a snack but some couples would bring carry-out and eat during the assessment time. Our challenge was that only one facilitator was willing to do an evening session. She only worked one evening a week so we could not always offer an evening group and it was difficult to find a sub for that facilitator. We also have turnover in our MA staff and keeping someone consistent for the evening clinic time was a challenge.

Hello,
Currently we only hold Centering in the evening in our practice (Monday - Thursday, 6-8pm) and it has worked really well both for our practice needs as well as for patient needs. The demand for Centering just wasn't enough to sustain daytime group sessions (we tried it a couple of years ago and discontinued due to low enrollment). We find that patients really like the availability of evening sessions. Hope that helps!

Healthcare is becoming increasingly like retail, you need to be open when your customers are available. Remember when banks were only open from 9-4 on weekdays? I don't know of any banks that actually keep "bankers hours" anymore.

In my journeys, I see sites successfully doing Centering at different times of the day, depending on the patients they are serving. Ranging from early morning through late evening. It never hurts to survey the moms in your practice, or schedule some different options. When I was at VCU we found that evenings did not work, and they were a strain on the clinic, and we had to start later in the morning or earlier in the afternoon to avoid school bus drop off and pick up times. Also there is always the question of how they would take time off to get to care outside of group? Are you offering evening hours? I have not worked with a population of factory employees, do they work shifts?
On addressing employers in the community... This is an excellent idea, and certainly worth considering, especially if there are one on two large factories and most moms work there. Anyone else have thoughts on this?

I am interested in Centering because: I love seeing villages come together. I love seeing new parents uplift each other and continue to do so beyond the immediate postpartum time.

1. Has anyone hosted an evening Centering group? We just started offering 'evening' groups. I put the evening in quotes because the groups meet 430-630p which isn't really evening. Some of our midwives jokingly call those groups our 'happy hour' groups. We had to offer it at that time because we utilize the 2nd MW to help with assessments. Our practice shares call and so having a clinic MW help with assessments helps our patients meet more midwives. Anyways...we offer the group at 430 so the clinic MW can help at the end of her clinic day. 2. If so, did you find there was a high no show rate or did the patients attend regularly?

Ohhhh good question. I will have to ponder this one...

I have run evening groups in the past and it was a great success! Patients were highly satisfied and I had a great attendance rate. It helped with recruiting because it works with many people's schedule. Childcare was sometimes an issue but some couples made it work by dropping kids off at a relatives house (since most people are out of work in this time slot). We did groups 5-7 or 6-8. The 6-8 groups were preferred by patients. I found that the one drawback to evening groups was facilitator dissatisfaction because it was hard to get into the rhythm/lifestyle of an evening group. I also trialed a Saturday group because patients were always asking for Saturday appointments. I found that attendance at this group was irregular because people would forget to come or have other life obligations. It was very difficult to find a consistent group of Saturday facilitators.

I have wondered about Saturday groups. That is good information to know. We will be looking into evening groups soon. Thanks everyone for your feedback.
