We are excited to announce that CHI recently awarded customized implementation support and group facilitation training service packages to the following six health centers:
Elements of the award may include electronic project management support to lead the health center through the planning phase, an on-site Kickoff Day with a CHI consultant, a Basic Facilitation training and Site Accreditation for quality assurance.
CHI remains committed to expanding access to the Centering model of group healthcare in communities where group care can have the greatest impact. Sites interested in expanding their Centering practice are invited to explore current funding opportunities available on our Implementation Awards page.
- Hawaii Health Systems Corporation - The Clinic at Waimea in Kauai HI
- The EpiCentre, Charleston SC
- STRIDE Community Health Center, four locations around Aurora CO
- Anne Arundel Medical Center - Women's Collaborative Care, Annapolis MD
- Women's Group of North Florida, Gainesville FL
- Tiburcio Vasquez Health Center, two locations in Alameda CA
