Does anyone have a pre-visit planning worksheet that they use and would be willing to share? We are doing fine planning the group content and activities, but what takes time and then takes away from group time is addressing care gaps for the women, such as following up on labs that weren't completed. We'd like to be more proactive about the clinical care and hopefully that way we have more time for the fun stuff. Thank you in advance!

I have a prep landing worksheet that I can send to you. Simply, there needs to be a pre planning session set aside to review the charts with your co-facilitator. It can not be done during group time . I will send you my template.

I just prep by listing all my patients names on a piece of paper with the super vital info in one column (like Gs and Ps, EDD, med rec number and major problems). Then on the second column I list what is up for them (like they need their result for their urine C+S or they ned their 3rd tri lab slip or they need a MSW appt). I keep that list to do follow up in between groups. Then I make the changes with the new TO DOs for the next visit and that becomes my new prep sheet. It helps me to have this sheet so I can see everyone at a glance. I am still not fast enough with EMR to do everything on the mat, so I need this piece of paper.
I just started doing it this way since we recently went to EMR, so I don't have a template. I am doing this with 3 existing groups of mine. It would take me some time to make one up. (the papers I have have private info on them, so I cannot share). I will share it later when I get a chance. I hope that is helpful.

Thanks, Laurie. This is what our providers do, as well. We recently switched to EMR, too, & it has been a little slower for all of us. I would love to see the template, too if it can be posted! Anything to make the providers' workflow easier Thanks, Kim

This is the sheet I use to track labs and medications. Some of the providers will even write the Centering session date they are due for certain labs in the blanks. We have a sheet that each patient uses to check in we review her previous visit and write important information to share with her or each other. When she has tummy time she leaves this with the provider who makes notes on it if necessary. We then use it after centering to do our documenting in EHR. We have found this very successful.

Hello All! Here is my Centering Group Worksheet. You can save it and make adjustments as you need or want after you save it to your system. I have found that basically, Sessions 1-4 are the prenatal visits that have the most needs i.e. labs, ultrasounds, individual special health needs which I put in under "other needs" One important point is that is document falls under HIPAA and will need to be protected. The pt leaves with the provider at Tummy time and after we put all info into EHR after centering sessions we shred.
