Group intake visits

For the sites that are doing the group intake visit, how are you handling the confidential/social history for the patients?  Do you have a form that you have them fill out and turn in for review?  Do the providers collect this information at their initial provider office visit?  We are strongly considering trying this model for our intake visit but are trying to work out the confidential/social history portion.  We are meeting a little resistance from a couple staff members here at the office in relation to this portion of the intake so we were looking to see what other centers do and how it is handled.  Thank you in advance!
