I am interested in Centering because: I feel it is the best way to get pregnancy care! Especially seeing how teens are with the model!

We have a welcome letter that will get sent to patients 1-2 wks prior to the first session. It states all of the information you have on your appointment template and it includes the first date/time. When a patient is scheduled for a group, we give them an appointment card with all the dates for their group.

I am interested in Centering because: I want to make a difference in someones life for the better and help educate them about the better way of healthcare. I am passionate about what I do with our Centering Program at Kaiser Permanente. I love the lifelong friendships that I build while going thru the journey of pregnancy for all of my patients. I love the personal care that patients receive and I love to keep our groups fun and current. Coming up on being accredited for a year in Jan 2018 and we have grown so much. Already starting 2 groups a month starting Dec 2017, beat our goal to start 2 by a month. #MicDrop

I think I may have posted what I thought of and made up myself after seeing what was being used at my clinic, I will post again tomorrow so you can see what I do. Hope it helps the patients and staff really like it!!!! Good afternoon!

So here are some appointment sheets I have made up myself to give to patients, I make sure they are different every month, and the recruitment sheet which is sideways again (I have no idea how to turn it right side up, its not like that in my phone or in my photos, so I am sorry you have to look at it with a possibility of getting a kink in your neck) I like things organized and I like to know what I am doing from one appt to the next and so do our patients. So I whipped these beauties up and put all of the Centering dates on them and the patients as well as the providers grab one and that way they can put it up and look on the appt sheets as to when their next session is and what number it is, so we will roughly know what we will be talking about, we all know it could be a totally different story when Centering starts depending on what the patients have on their mind that day. I find these helpful and I think you would too! you could always just trial it too with one group or something and see how it goes, but this way the patient knows , the providers know, and more importantly you know when these dates are. Organization, consistancy and communication is key. Since I am the Centering Coordinator for our clinic, when a new group is about to start I cut and paste on of these sheets with the dates of the group that is starting and I send an email with this sheet to the admin assistant, she does the staff scheduling, I send a copy to the Conference Center, since we don't have a designated Conference room, I took the role of making sure we had a conference room set up, I call every Monday to the conference room scheduler, even tho I had sent a copy of the schedule to them I call them EVERY Monday to confirm they have us down for our dates that week that we have Centering. Since I took over that role in January things have been pretty much flawless. Sometimes too many cooks in the kitchen doesn't make it a better dinner if you know what I mean. I like to simplify things, and being the Centering Coordinator, I feel the bulk of the Centering stuff from recruiting, flyers, shopping for Centering supplies and snacks, ordering educational materials ect to being backup Co-Facilitator for the new groups starting because we are down 2 MA's. I communicate with the facilities manager directly, any department I have to deal with don't like to leave messages or talk to 4 different ppl before I get a correct answer ( some may think thats pushy, but I only have good intentions for the patients and if I am frustrated by needing to jump thru all these hoops to get an answer or whatever and I work there, I can't imagine what would happen if a patient needed something regarding Centering Pregnancy and had to jump thru a ton of hoops, they would think it is too complicated and they would have a negative experience about Centering before coming to the first session) so I go directly to the source, the manager, the upper manager, the lead midwife, the lead doc ect. I would rather take it all on myself or the bulk of it because I know I will be persistent and will get things done, I also give a copy to the nurses, sometimes they add patients to the daytime schedule or to our MA schedule so this way they know that these Centering days we need consistency, consistency for us, our families, our patients and our sanity. Lol I hope you find some ideas that help in this post and again I am sorry it is long.

We use these and check off the group based on due date I agree with Paula, this is a really nicely designed information sheet.

Nurse Sessoms You can save them as a PDF in Microsoft Word/Powerpoint. Click "Save As" and then there's a pull down menu for format. Click the file type (PDF) and it will save the file as a PDF. SaveSave
