Does anyone have a pre-visit planning worksheet that they use and would be willing to share. Something like a one page flow sheet. I have found several files designed for CenteringPregnancy, which I could modify. Is anyone finding that they need this for CParenting groups? We are just starting CParenting groups and I assume it would be helpful, as it is in Pregnancy Groups. We do not have a pre-group worksheet , but I look ahead and see what immunizations or blood work they need and what medical problems they have and just write it in on my schedule for that day !!!

This is the form we use. We fill it out as we go along and keep it in a locked cabinet when not being used in centering then we shred after session # 10

Wenonah (she/they) and Andrea CCFT, thank you both for posting these documents. A question for both of you, do you have these templated in your EHR so that you can simply print them out for each patient prior to each group session? Andrea CCFT suggests that this is a manual process - that a single copy of the patient record is kept locked up.

We do not have them templated in EHR. We keep locked in a file and add to them each visit after the reunion we shred them.
